I authorize TapeCenter to repair the enclosed tape(s). I understand that damaged portions of my tape may have to be edited out and/or it may not be possible to save the original shell or labels. TapeCenter is not financially responsible for any customer's tape lost or damaged in return shipping. TapeCenter bears no responsibility for customer's tapes other than proper repair and return shipping. No dollar value may be assigned to any tape lost or damaged. TapeCenter is not responsible for the contents of any tape shipped to us and will comply with federal and state laws regarding material deemed adult in nature. TapeCenter will comply with all federal and state copyright laws and may choose to reject a request for tape duplication. TapeCenter retains the right to accept or reject any tape submitted for repair and/or duplication. Copies from VHS-C, DVM, and 8 MM tapes will be made to standard VHS tapes. My signature represents my agreement with these terms and conditions.
